Notice to Architects, Engineers and Surveyors
Notice is hereby given that the City of Forks, pursuant to RCW 39.80.030, is establishing a roster of professional services to run from 1 January 2012 through 1 January 2013. Specific project information will be provided to those firms that are the most qualified in a given specific profession.
The City of Forks solicits interest from consulting firms whose practice including one or more of the following specialties and who would like to be considered for projects:
Architects
Airport Facility and Infrastructure Engineering
Civil Engineering – Streets and Storm drainage
Construction Management
Drafting Services
Fisheries Related Permitting
Electrical Engineering
Geographical Information Systems
Graphic Design
Hazardous Materials
Hydrologist
Landscape Architecture
Materials Testing
Mechanical Engineering
Pavement Analysis
SEPA/EIS Analysis
Structural Engineering
Surveying, Mapping, and/or photogrammetry
Traffic and Transportation Engineering
Wetlands and Critical Areas Analysis (Steep Slopes, Shorelines, streams, etc.)
In addition, the City may also utilize this list to acquire the services of a “clerk of the works” for federal, state, and/or locally funded construction projects occurring in 2012. In addition, if permitted by rule, regulation, and/or law, this list may be utilized to solicit for the above listed services associated with projects funded by any federal and/or state grants received by the City in 2012. The City may also utilize this list to consider acquiring services for any FAA funded airport improvement projects at Quillayute Airport (UIL) that could arise in 2012.
Further, these services may be funded or partially funded by grants or loans from the U.S. Department of Housing and Urban Development (Washington State CDBG), U.S. Department of Agriculture-Rural Development, U.S. Department of Transportation, Washington State Department of Health, Washington State Department of Transportation, Washington State Department of Ecology, or any other such agencies.
Submittals should consist of no more than three total pages per firm with the following format: Cover letter clearly specifying, within either the subject line of the letter or the first line of the text, the specialty areas for which the firm would like to be considered based upon the listing given above. Cover letter should also give a brief description of the firm, its size, familiarity with government projects and funding thereof, and experience with working with small municipalities with less than 4,000 in population. Cover letter should include all relative information regarding address, phone, fax, and applicable e-mail address by which the City may contact the firm by phone, letter, fax or e-mail asking for additional qualifications regarding a specific project.
The Selection of a consultant for a specific project will be based upon the following criteria areas: Key Personnel; Experience; Demonstrated Interest; Availability; Ability to meet project schedule; Firm location; Past performance; In-house expertise; Familiarity with applicable standards; Special Qualifications; Past experience on small municipal projects; and Minority and women business involvement/enterprise approach. The City encourages minority and women owned firms to respond, and does not discriminate on race, color, religion, age or gender.
The City of Forks assumes no obligations of any kind for expenses incurred by any respondent to this solicitation.

An ordinance annexing property into the City of Forks.
Dahlgen-Pleines-Saunders. Annexation
WHEREAS, property owner’s of over 10% value of the property described below notified the City Council of their intent to commence annexation proceedings for said area: and
WHEREAS, during. that meeting the City Council did determine that it would accept the proposed annexation. In addition, the Council determined that it would require the simultaneous adoption of the Forks Zoning Code and Zoning Map upon the proposed annexation area. Finally, the Council determined that it would not require
the assumption of any part of, existing city indebtedness by the annexed area; and,
WHEREAS, the City Council received a petition for annexation of the area signed by Fred & Lorraine Pleines, Joel Dahlgren, and Pete & Cheri Dahlgren who are the owners of over 60% of the value of all of the land in the area to be annexed; and
WHEREAS, the Clallam County Boundary Review Board took no action on the petition after said petition and associated documentation was forwarded to said Board for their review and languished there without review or action;
WHEREAS, the City Council held a public hearing on the petition on 24 October 2011 and determined that the annexation would be in the best interest of the community and that the annexation should take place,
WHEREAS, the City Council received a petition for annexation of the area signed by the owners of over 60% of the value of all of the land in the area to be annexed; and
WHEREAS, the City Council had scheduled a public hearing on 24 October 2011, but rescheduled that hearing to 14 November 2011 as a result of communications from Fred Saunders on behalf of the Saunders Family, City staff wanted to affirm that (1) the Saunders did in fact own a parcel earlier identified as being owned by the Dahlgren fmtily; and, (2) that the Saunders did not object to be included in the
proposed annexation;
WHEREAS, the City staff learned from Fred Saunders that a recording error within the County’s database had erroneously identified one parcel, amounting to approximately 2.5% of the total value, as being owned by the Dahlgrens when in fact it was owned by the Saunders family; further, as the representative of the Saunders family, he indicated that there was no opposition to the parcel being included in the proposed annexation;
WHEREAS, pursuant to the revised public notice, the City Council held a public hearing on the petition, after notice was published hi the City’s journal of record and sent to adjacent landowners and Clallam County, and determined that tax revenues would be increased by the annexation and that the annexation should take place:
BASED UPON THESE FINDINGS, The City Council of the City of Forks do ordain as follows:
Section 1. The following described real property contiguous to the current limits of the City of Forks shall be annexed to and become a part of the City of Forks:
The NW 1/4, the SW 1/4, and also the remaining portions of the SE 1/4 currently not within the City limits within the NW 1/4 of Section 17, Township 28 North, Range 13 West. W.M., of Clallam County;
And,
The NW 1/4 of section 17, Township 28 North, Range 13 West, W.M., of Clallam County.

Section 2. Assumption of Indebtedness. The property described in Section I and annexed into the City of Forks shall not be required to assume any past indebtedness of the City of Forks,
Section 3. Assumption of Forks Urban Growth Area Zoning Code. The property described in Section 1 shall be zoned in the manner noted within the Forks Zoning Code and the map that accompanies said code.
Section 4. Effective Date. This ordinance shall take effect five days after the publication of this ordinance within the Forks Forum.

Notice to Contractors
Establishment of a Small Works Roster for 2012
Notice is hereby given that the City of Forks, pursuant to RCW 35.23.352, 39.04 et al, and City Ordinance No. 475, is establishing a small works roster of responsible contractors properly licensed and registered to perform the work for which they would like to be considered by the City when projects arise.
The City of Forks solicits interest from contractors with the following trades or skills:
Construction
Carpentry
Plumbing
Electrical
Excavating
Foundation and cement work
Paving
Flooring
Road maintenance and painting
Telecommunications, to include network cabling, installation and repair
Heating, ventilation, and air conditioning
Gas (propane and diesel) heating
Septic/Wastewater hauling, mainline installation and/or repair
Logging (failing, busheling, etc.)
Log hauling/transportation
Metal fabrication and welding
Submittals should consist of:
• Cover letter clearly specifying, within either the subject line of the letter or the first line of the text, the specialty areas for which the contractor would like to be considered based upon the listing given above. Cover letter should also give a brief description of the firm, its size, familiarity with government projects and funding thereof, and experience with working with small municipalities with less than 4,000 in population.
• Copy of current state licenses, certifications, and/or registrations;
• Copy of current bond and/or insurance if applicable; and Three references.
The City will utilize the small works roster for projects up to $200,000 in total costs (labor, material, equipment, sales and or use taxes as applicable). In addition, the City may utilize this list for logging jobs/operations involving City owned property. The City will utilize either written requests or phone requests for bids when projects arise. Awards shall be made pursuant to Ordinance No. 475 – available upon request.
The City of Forks assumes no obligations of any kind for expenses incurred by any respondent to this solicitation.
