Everyone in town should be very proud of our Long Term Care Facility at Forks Community Hospital. For the third year in a row they have had deficiency-free survey inspections from the Department of Social and Health Services.

It is very rare for a facility to have one perfect survey, let alone three in a row! This is really a reflection of the hard working, caring and compassionate staff that provide much needed care to our community’s most vulnerable adults. Not only has the facility had three perfect surveys but it also has five star (top rated) ratings in Quality Measures, Nurse Staffing and Overall Rating.

What an honor to have such a top rated Long Term Care facility in town!
